Back when I used to play Duke Nukem and Unreal Tournament, I worked out I could have my right mouse button work as "w" or "run" and put alt fire as "w". That way I could run across the map and type a message at the same time. With it came many other benefits. From the keybinding Geeknights episode, I think you do the same, right Rym?
Scott, I think you're being needlessly pedantic about what constitutes cheating. It's one of those common situations where I find myself agreeing with much of what you say, but disagreeing with your conclusion.
I think it's pretty fair to say that, no matter what the game, there are some "rules of fair play" involved, and breaking those rules is equivalent to cheating, even if you want to give it another name. If I sit down to play a game with you, it's to be assumed that I'm the one playing and making the decisions in the game, not the guy across the room with an earpiece in my ear and a microphone in his wrist. If you want to argue that that's not cheating, but some other thing that you want to invent your own term for, that's your business.

I try to modify the defaults as little as possible, so my setup is normally more like:
LMouse - Shoot RMouse - move forward MMouse - switch to secondary weapon (I use this in NS when I run out of bullets with the LMG or HMG, to bring up the skulk-killing pistol) MWheel - I hate mouse wheels.
a - left d - right s - back w - secondary fire (this is reload in NS for marine, and movement for alien) e - use r - reload tab - crouch space - jump q - cycle weapon c - voice chat v - other
With this I can press pretty much every button without lifting my finger or thumb OFF of a different button. Meaning

Took me a second to think of a good response to this, and here it is: It's cheating if 1) there's no skill involved (shouting BLARGHHAHAHAHAHAHAHA in someone's face to distract them while they're trying to take a move) or 2) if there is skill involved, but the action violates the rules of the game (using slight of hand to switch a card in your hand and a card in your discard pile in Dominion).

This is perfectly AOK unless the game has specific rules against it. Look at sports, especially the NBA. While they play they trash talk the fuck out of each other to mess each other up. Perfectly OK as long as they don't get a technical foul. Perfectly OK in the NFL also, as long as it doesn't cross into unsportsmanlike conduct. Also look at people in the stands during field goal kicks or free throws trying to wave things to create visual distractions to mess up the player. Totally legal. It only becomes interference if a fan physically interferes with the game by touching players/the ball/the field of play. Or how about poker, where people will say absolutely anything to mess with the other players. In golf it's not ok because it has rules requiring silence.
Space. I'd like to swap it with something else, but I have nothing sufficiently important in the FPSs I'm currently playing, and I have vested muscle memory in my current configuration.
