I agree with Pete on that front. I don't think it is very stand out even as a pop song. I very much think your fourth point is the strongest in that it associated a dance with a somewhat catchy song. Three holds up to a lesser degree. If we look at the Macarena as a reference point it's not so much about the song being good at all but that there is a dance that is easy to pick up and is silly.
It's not exactly what I would call fun, but I can understand why other people would think it's fun.
You're right, that it doesn't stand out among pop songs, music or video. It's just that it meets the requirements for these items. By meeting each of these checklist items, it exploded.

Well, you said "well-done" and "catchy." I take that to mean "moreso than many pop songs." I disagree with that assessment.
I think the absurdity of the video, and the "give no fucks" attitude of the dance woule be more significant factors.
There are lots of chiptune covers of Gangnam Style, as ProfPangloss and I found one evening, but most of them really suck. This one is the best (via the always awesome /r/chiptunes):
I had the honor of seeing both of these men (separately) perform due to my seasonal work at a local concert venue.
While I have no personal anecdote about Psy, I can tell MC Hammer is a dick. He didn't like waiting like a normal human at a security checkpoint we'd set up so he had his driver blow past it. It was a fairly large deal for us.
Yeah, dude. You're "talent", we get it. But act like a normal human. Everyone wants to be safe.
Related to that my friend's father is a director of art in the new york new jersey area and had a job working with Martha Stuart once. Lets just say that her staff are utterly terrified of invoking her wrath.
Was at an AHL hockey game. They played this so much. Incredibly surreal. Some kind of cyberpunk situation where the Internet had breached into the real world.

If he wins, I want to see him do it at the Inaugural Ball.
Well, humanity just peaked. It's all downhill from here.

(I have a macro to take me to unblocked versions of any video)
Also, Scott may note - During the chorus, the arch lights move to the correct left-right-left pattern.
